Carers

Are you looking after a relative or friend? Then you are a carer.

In order that your GP can do all he can to help, it is important that we are aware of your status as a carer. The practice will not be able to share any clinical information with carers without the written consent of the patient concerned by downloading and completing our Carers Consent Form or asking at reception for a carers form to complete.

CARERS – CONSENT FORM

Swansea Carers Centre

Swansea Carers have developed new services during the pandemic.

Family carers have taken on huge challenges as a result of the COVID-19 pandemic. Many have had to provide more care while dealing with financial pressures and increased levels of isolation.

Swansea Carers Centre has continued to provide much-needed services, in line with government guidelines, on the phone and via Zoom. Our Counsellors have been meeting clients 1-1 because of the increased demand for counselling and the welfare benefit teams have assisted carers with applications for benefits on the phone.

During the pandemic, we increased our services by employing new staff to:

  • Work specifically with Male Carers
  • To work within the BAME community,
  • To increase our dementia support services
  • To increase our Counselling service
  • We have extended our advice and information helpline from 4.30 until 8pm
  • We have introduced a Mental Health Well-Being support line

Help and support are available for Carers at Swansea Carers Centre:
